Waiver
As a volunteer, you must be willing and able to carry out your assigned duties and responsibilities. You must be reliable and committed. Duties are matched to the interests and abilities of the volunteer.
By checking the box below I affirm that:
I and/or my child or children are able to participate as a volunteer in all activity for the Event and agree to do so at my or our own risk.
I understand that volunteers are prohibited from carrying or using firearms during the Event and/or on City property (including City vehicles). Volunteers who hold a license to carry must keep such items in their locked personal vehicle. Firearms and ammunition are not allowed in City vehicles.
I agree not to consume or use tobacco products on any volunteer assignment.
I agree not to consume, use, possess, or be under the influence of any drug or alcohol products on any volunteer work assignment.
I/my child agree that I/my child serve on a voluntary basis and not as an employee, contractor or agent of the City of Richardson.
I understand that any pattern of conduct that would tend to disrupt, diminish or otherwise jeopardize public trust in the City of Richardson will result in dismissal.
I understand that my volunteer assignment with the City of Richardson may be terminated at any time.
I understand my involvement with the City of Richardson may include coverage in the media. I approve the use of my likeness, voice, photograph, words and any other creative work without payment or consideration by the media or the City of Richardson.
WAIVER OF LIABILITY
IN CONSIDERATION OF THE CITY OF RICHARDSON ALLOWING [ME] [MY CHILD/CHILDREN] TO PARTICIPATE IN THE EVENT, AND BEING AWARE OF THE POSSIBLE INJURIES THAT COULD OCCUR AS A RESULT OF THAT PARTICIPATION, I, [ON BEHALF OF MYSELF] AND/OR [MY MINOR CHILD/CHILDREN] RELEASE THE CITY OF RICHARDSON, ITS OFFICIALS, EMPLOYEES AND AGENTS FROM ANY AND ALL INJURIES AND DAMAGES WHATSOEVER ARISING FROM PARTICIPATION IN THE EVENT.
I, MY HEIRS AND REPRESENTATIVE, AGREE TO INDEMNIFY, SAVE AND HOLD HARMLESS THE CITY OF RICHARDSON, ITS OFFICIALS, EMPLOYEES, AND AGENTS FROM ANY CLAIMS MADE BY ME (MY CHILD/CHILDREN) OR MY INSURER FOR INJURIES OR DAMAGES RELATED TO THIS EVENT.
I UNDERSTAND THAT APPOINTMENT TO A VOLUNTEER POSITION MAY BE CONTINGENT UPON THE COMPLETION AND REVIEW OF A CRIMINAL BACKGROUND CHECK.
